Michael Stenstrom investigated cause of Hyperion sewage spill

Synopsis: Stenstrom was an investigator on the Hyperion sewage spill. In an official report, the investigation team concluded that the spill was caused by systems failure and human error.  

UCLA News: Stenstrom's original hypotheses were shown to be incorrect once investigations began. “He also initially assumed the cause of the spill was outside debris. But when he and others started investigating, they knew that wasn’t the case."
